In order to obtain further supplies of your regular medication, please either:
Post the request in the prescription posting box on our front door
Fax the request to us on 01694 724604
Post via Royal Mail
Request the medication via Patient Access
Email us at churchstrettonmedicalcentre@nhs.net
Please note that we do not take prescription requests by telephone
We would prefer if you could request your medication using the right hand half of your previous regular medication prescription.If the drug you require is not on the sheet, you will need to speak to our prescription administrator who is only available in the mornings. She may have to speak to the doctor about your request.
Repeat medication is reviewed on a regular basis and you may be asked to make an appointment with a doctor or attend a clinic before another prescription is issued.

The practice and the local pharmacists offer a service to enable patients to collect their medication from the pharmacy of their choice, in Church Stretton, without having to collect the prescription from the practice first.If you wish to do this, please inform us and state which pharmacist you prefer to use.
Medication Reviews
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
