New Patient Registration

 

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please ask our receptionists for details of the practice boundaries.

 

On registering you will be asked to complete a new patient form for each member of your family. If you wish to prepare these in advance, please print off one of our registration forms below and bring it to the surgery:

 

 

 

 

 

 

 

 

Please register with us as soon as you have moved into our area and try to avoid registering with us at short notice, as your previous medical record will take time to reach us. Your records may contain important medical information that will help us to make clinical decisions that are best suited for your care.

 

If you are taking repeat medication of 4 or more items please book an appointment with the practice nurse for a new patient check. We will not be able to issue you with any medication until a doctor has seen you.

 

Since the government changes to General Practice in April 2004, patients are registered with the practice rather than a particular doctor. If you have a preference to which doctor you see, this can be recorded when you register. The practice will endeavour to give patients their choice of doctor but this may not always be possible.

 

You can also ask to see a particular doctor when making an appointment. This will be accommodated where possible, but it may mean waiting longer for an appointment.

 

It would be of great help to us if you would kindly provide us with patient telephone numbers (land line and mobile) and next of kin information in case we have to contact you for any reason.

 

Medical treatment is available from the date of registration. Please contact reception for further information.

 

Moving out of the Practice Area

 

If you move away from the practice area, don’t forget to register with the practice which covers your new home. This will ensure that your medical records are sent to your new practice and will help them to provide the medical care which you may need.
