Non-NHS Services

 

Our contract with the NHS covers the provision of sickness certificates for people who are employed/unemployed. Most other certificates and forms are not covered by the NHS. We provide these as a private service and charge at a cost which reflects the time taken. Please ask a member of staff regarding the fees chargeable if wish to know the cost for completion of a particular form.

 

Some examples of Non-NHS Services include the following:

 

Medicals for pre-employment

 

Sports and driving requirements (HGV, PSV etc.)

 

Insurance claim forms

 

Holiday cancellation forms

 

Pre-employment health forms

 

Prescriptions and/or Letters for taking medication abroad

 

Letters "To Whom it May Concern", eg fitness to use a gym

 

Forms/letters regarding collection of rubbish

 

Private sick notes

 

Vaccination certificates

 

Please note we do NOT sign passport forms

 

We would normally ask you to allow 10 working days for completion of forms and letters such as those listed above (although it will normally be much quicker) as these are not classed as a clinical priority.

 

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

 

Photocopies of letters/documents are charged at 35p each.
